If you feel sick or sustain an injury because of your work environment, tell your administrator that you need to go to Occupational Health to document your symptoms and get assessed. Your administrator cannot deny your access to Occupational Health, nor can they tell you to wait and go after work. 

On your way to Occupational Health, call HR to document your illness or injury due to your work environment at (475) 220-1540. If no one picks up, leave a message.

Workers’ Compensation​:
Fill out a 30C Form and send it to Barbara Mincey in HR ([email protected]). Follow the directions on the form for mailing a hard copy.​
