If you need to take a Leave of Absence (LOA):

Step 1: Initiate Application of LOA (Everyone requesting a leave of absence must complete this form)

Step 2: Please complete ONE of the following:

• FMLA Application for Employee/Self (Use for Maternity and Medical leave of absence)
• FMLA Application to Care for a Family Member (Use this for Paternity Leave and all other family medical conditions)
  
  

Step 3: Return to Work Authorization (Must be completed by you and your doctor prior to returning to work)

• FMLA Return to Work Form
